Under the bylaws of the Canadian Music Centre (CMC), Members and Associate Composer Members may vote by proxy should a member be unable to attend the 2023 Annual General Meeting.

Any such proxies should be in the hands of Heather Pawsey, CMC Chair, before the start of the Annual General Meeting on Saturday, June 17, 2023 at 3:30pm at Canadian Music Centre (20 St. Joseph Street, Toronto, ON M4Y 1J9).

Please submit this form to the National Office by email: info@cmccanada.org.

Each proxy shall apply to the Annual General Meeting (AGM) of Saturday, June 17, 2023 and must name the board member to whom the proxy is given, and must be signed by the person giving the proxy.
